Eligibility for Forbush Memorial Library Card
Forbush Memorial Library cards are free to all Massachusetts residents of all ages. Out-of-state visitors may receive temporary cards - see our Library Usage By Guests policy for details.
Current Cardholders
Forbush Memorial Library is a member of the CW MARS library consortium. If you already have a library card from another CW MARS library, you can use it at Forbush.
If you have a card from a non-CW MARS library, you will need to sign up for a CW MARS library card in order to use our library.
Complete an application form
You can fill out a library card application form in-person at any circulation desk. You can also print it out at home and complete it ahead of time.Present Photo ID and proof of address
Complete your registration by providing photo ID and proof of your current address along with your completed form.
If your Massachusetts License or State ID lists your current address, that’s all you need. If it does not, you will need to bring an additional document (such as a rental agreement, bill, or piece of mail) showing your name and current address.
Youth (ages birth to 18) can receive a library card after a parent or guardian with proper identification completes an application form.
CW MARS library cards must be renewed regularly by a librarian to confirm your account and contact information. You can renew your card at any CW MARS library.

The Board of Library Trustees reserves the right to deny library services to residents from any municipality whose public library has been decertified by the Massachusetts Board of Library Commissioners (MBLC).
Accessing Your Online Account
Your CW MARS online account allows you to place holds, pay bills, change your notification settings, and more.
To log in, you will need your library card number (or user name) and your password. Your default password is the last 4 digits of your phone number. If you need to reset your password, please contact the library.

If you would like to access CW MARS’ extensive digital collections, you can make a CW MARS eCard. The eCard can be created entirely online. Learn more and apply for your eCard here!
An eCard can be upgraded to a physical card with full borrowing privileges by visiting us at any time.
All Massachusetts residents are eligible for a Boston Public Library (BPL) eCard, which provides access to all online resources, including Hoopla! Hoopla has eBooks, audiobooks, music, movies, and more. Find out more on the BPL website.
